Ten Hag is keen to overhaul his attack, and it is believed the Red Devils are planning to buy Ramos as well as their No. 1 target Harry Kane from Tottenham in the summer transfer window.
Ramos has scored 17 Primeira Liga goals this season and 25 in all competitions, and he will cost around £100 million.
The 21-year-old displaced Cristiano Ronaldo from the Portugal side at the Qatar World Cup and impressed with his all-action performances, scoring a hat-trick against Switzerland in the last 16.
